Form for 3D printing at Burlington Public Library

  1. Burlington Public Library 3D Printing Agreement
    Please read and note your agreement to all the following conditions of use.
  2. 1. The public will not be permitted to use the library's 3D printer to create material that is:
    A. Prohibited by local, state, or federal law. B. Unsafe, harmful, dangerous, or poses an imminent threat to the well-being of others, including but not limited to drug paraphernalia and weapons. C. In violation of federal, state, or local laws or regulations regarding accessing, viewing, printing, and distributing obscenity or child pornography. D. In violation of the City of Burlington's policy on sexual harassment. E. In violation of another's intellectual property rights or the 3D printer design software terms of use, For example, the printers shall not be used to reproduce material that is subject to copyright, patent, trademark, or trade secret protection.
  3. I understand and agree to abide by the conditions stated above. (1)*
  4. 2. The library's right of refusal.
    The library reserves the right to refuse any 3D print request.

  6. 3. Item pick up and file deletion.
    Items printed with the library's 3D printer that are not picked up within 30 days after completion and notification will become the property of the Burlington Public Library and the patron waives all property rights in the printed object. Design files will be deleted by library staff at the time of printing or at the end of the day. Items must be picked up by the patron who submitted the file(s) for printing unless alternative arrangements have been made in advance.

  8. 4. 3D print items intended use.
    The library's 3D printers are intended for educational, entertainment, and prototype purposes. The library does not provide access to 3D printers for the production of goods intended for sale. Assistance with the use of the 3D printer by staff does not constitute knowledge, or acknowledgement, of any final use of the 3D object. Once in possession by the patron, the library expressly disclaims any and all personal injury or property damage caused by use or misuse of a 3D printed object.
  9. I understand and agree to abide by the conditions stated above. (4)*
  10. 5. Final agreement and understanding.
    I have read and understood the 3D printing conditions above and I have no questions as to what these conditions mean. If I had any any concerns or questions, I have contacted the Burlington Public Library staff and my questions have been answered and explained to me to my satisfaction.
