Community Memories

  1. The Burlington Public Library is launching a project to collect the stories of how our community is living through this truly historic time. We want to learn the stories of how we all—as individuals and as a community—are reacting and responding to the 2020 COVID-19 Pandemic. The library will archive your experiences, to insure that this community account is available to future generations.
  2. We are asking agencies, organizations, and individuals to share their pandemic stories through media. You can submit all forms of media, including but not limited to: stories, photographs, music, poems, artwork, and so much more.
  3. Please read each statement and check the Agree box below to indicate your understanding and agreement.
  4. • I understand that the materials I am contributing today to the Documenting COVID-19 Community Memories Project will be maintained by the Burlington Public Library (BPL). I understand that the material/file(s) will be managed in a manner consistent with established professional standards and BPL collection development policy and I place no restrictions on their use or disposition.
  5. • I agree that BPL may make publicly accessible, either immediately or in the future, the material/files I’m submitting for this project.
  6. • I understand that material submitted through this form must not violate any confidentiality, privacy, security or other laws. I agree not to submit material that contains sensitive personal information about other individuals, including personal health information.
  7. • I give consent to BPL to digitally reformat the collection or migrate existing digital content to new technical environments as appropriate for preservation and/or access purposes.
  8. • I confirm that I own (or have legal authority over) the materials I am submitting. I voluntarily transfer to BPL all rights, title, and interests, including non-exclusive intellectual property rights, to these materials.

  10. This could be biographical information such as age, gender, occupation, information about how the COVID pandemic has impacted you/your family or changed your routine. If the creator is an agency, business or organization this could be its name plus information about its mission, how long it has been in existence, the impact of the pandemic on it, how it is adapting with the changes, etc. (optional)
  11. Think about what someone 50 or 100 years from now would need to know to understand the context and content of the file(s). For example, give the date the item was created, as well as information about the place or organization, the names of individuals, and the reason why you feel this file helps convey the impact and experience of COVID-19 in the City of Burlington. etc. (optional)
